Exegy Diversity Survey

While looking at the available jobs at Exegy Inc, I noticed something very interesting about their current job application process, they have included an optional Diversity Survey for applicants to fill out.
One of the questions on this survey “Which of the following communities do you belong to?” has some interesting choices for a potential application to choose from like Person with Disabilities, Neurodiverse, Parent and Refugee or Immigrant.
You would think some of these choices would be illegal to ask, because it could raise a lot of discrimination concerns.

Let me try to break down each of the following choices, even though the survey states it wouldn't affect the person's job application.
Person with Disabilities
Is this something really important for a company to know before meeting a potential applicant in person.
Neurodiverse
Could the company be using this as a way to find someone that might be a good potential since a lot of bright people in technology are Neurodiverse or could they be using this as a well to filter out someone who might be a handful depending on their Neurodiversibility. According to the ADA, Neurodiverse is classified as a Disability, so shouldn't it still fall under the other choice “Person with Disability”…?
Parent
Why would this choice even matter to a company? The only potential reason for it would be to find out if this person might have more important duties over their job as being a parent first.
Refugee or Immigrant
This is something strongly discouraged by the EEOC and DOJ to ask, since it can easily lead to a claim of national origin or citizenship status discrimination

This survey is something new I noticed when they went to Paylocity for posting their job openings over to this embedded version on their website. I actually don't ever remember the company ever asking if I wanted to fill out this kind of survey when I applied there.
What do others thinks, should these surveys be illegal to ask during all job applications and offers…?
